The ultimate goal is to support the systematic adoption, implementation, and evaluation of successful programs, practices, and policy changes. The database provides carefully reviewed, documented, and ranked practices that range from good ideas to evidence-based practices.

Goal: The Children’s Health Fund’s mission is to provide health care to the nation's most medically underserved children through the development and support of primary care medical programs. The fund also responds to public health crises and works to promote access to health care for all children.

Impact: Children's Health Fund provides medical, dental, and mental health care to approximately 83,000 low-income and homeless children and their family members each year through mobile medical clinics.

Goal: A key goal of this program is to help clients continue to reside at home, despite serious chronic conditions.

Goal: The goal of this workshop is to build the participants’ self-confidence in their ability to manage their health and maintain active and fulfilling lives despite their chronic disease condition.

Impact: In 2016 Derby Health Collaborative hosted six free self-management education courses in partnership with the Derby School District, the Derby Senior Center, the Derby Public Library and the Kansas State Extension and Research Center. Workshops will continue in 2017.

Goal: The goals of this program are to identify children at risk for chronic lead poisoning, to decrease blood lead levels in chronically poisoned children, and to help families find stable, lead-free housing.
